When I teach system analyst and design, I always emphasize that when building a computer system, you have to put a "heart" in it. This mess above is one of the best example what happens when you design a system without considering the human factors.
Well, to quote a technical error is an easy thing, but what about the emotional factor. For those that thought that they were successful only to be told later it was a technical error. In US, I bet there will be multiple million dollars lawsuits already due to "emotional stress".
And how can there be a technical error of this magnitude? When I was in my previous job, I prepared data for the Penang K-ICT meeting for the Chief Minister. We checked the data like 7 times before we give to him as a mistake cannot be easily solved by saying I am sorry.
I hope all those high tech computer experts realized this...

NOOBSSSS??
I think, even being a noob is not a valid excuse...
And the emotional stress is not limited to the not-to-be undergrads. Their parents are likely disgraced, too. Imagine telling all your relatives and neighbours that your child is going to USM, only to have to tell them later it was a mistake (that your child did not qualify). It must be terrible.
I hope someone comes up with something more remedial than an apology before any of the "victims" turn suicidal...
